Permanently Disabled Voters

If a voter is registered to vote in Grundy County and is permanently physically incapacitated and unable to make it to the polling place, the voter is qualified to automatically receive an application for a Vote By Mail ballot before every election for the next 5 years.

An Application for an “Absentee Voter’s 5 Year Identification Card”, must be properly completed and signed by both you and your doctor once every 5 years. Click on the Download link below to print an application

Once the application is completed, return the form to the Grundy County Clerk’s office. We will forward the application to your doctor. The doctor will complete and sign their section on the reverse side. (If the voter chooses, they may take the application to their physician to complete. Return the form to the Grundy County Clerk’s office). Any questions, contact the Grundy County Clerk.
