Voter Admitted to Hospital, Nursing Home or Rehab Center

A qualified Voter who has been admitted to a hospital, nursing home, or rehabilitation center not more than (14) fourteen days before an election shall be entitled to personal delivery of an absentee ballot by making application with the Election Authority (10 ILCS /19-13)

This provision for absentee voting is available through Election Day, if the process can be completed. The Voter and their doctor must complete and sign a legal affidavit, which is available at the County Clerk's office. It serves as an application form that must be returned to the County Clerk's office.

The Clerk's office will then issue a ballot at that time to a relative (spouse, parent, child, sibling) or another registered voter of the same precinct who will deliver it to the voter who has been admitted. The individual delivering the ballot on your behalf must sign the affidavit accompanying your absentee ballot application.

The completed/voted ballot must be returned to the County Clerk's office before 7:00 p.m. on Election Day.
