If you are permanently physically incapacitated and unable to make it to you polling place, you qualify to automatically receive an application for an absentee ballot before every election for the next 5 years.
You must be a registered voter in Grundy County. An Application for Absentee Voter’s 5 Year Identification Card form (below), when properly completed by you AND your doctor, it may be of some help. By applying for the Disabled Voter’s Card, the doctor’s signature is obtained once every 5 years.
The Elections Department will automatically send you an application requesting an absentee ballot for every election in which you are qualified to vote.
Once you have completed your information on the form, you may return it to our office at the address below and we will forward the application to your doctor for his/her signature OR you may take the form to your doctor and ask them to complete their section and then return it to our office.
